Listed prices effective for events occurring from 2/1/2024 through 1/31/2025

Basic Assumptions
- Prices are based on single-worksite, single-day programs; multiple-day events are available (contact us for more details)
- Each program includes two to seven hours of service depending on the committed participant-minimum
- Online scheduling included with tablet-based biometric screenings (additional charge for paper-based screenings)
- Data is collected using point-of-care testing, producing results that are obtained and reviewed within minutes
- Tablet-based Health Risk Assessments (HRA) and biometrics with immediate electronic reporting is available
- Aggregate summary report or data file transfer included for all tablet-based programs (additional charge for aggregate summary reports for paper-based screenings)
- All Prices are before the applied discounts from the Year-Long Wellness Program (if enrolled)

There is no charge for travel within 125 miles one-way of Sacramento.
Costs for travel over 125 miles will be given upon price quote.
