• Free Sample for HR Representatives:

    • Up to 3 HR Representatives per company can experience the Emotional Well‑Being Assessment firsthand with the link above.
• Virtual Assessment Window: Each client chooses a week for their employees to take the assessment.
• Health Fair On-Site: Select the date and if you want CHA to host or for someone on-site to host. If you select an on-site host not from CHA, you will be provided a Personalized Flyer with a QR code for registration, an On-Site Representative Training Guide, and Participant FAQ.
• Service Includes:
  • HR Representatives will receive:
    • An introductory email template for employee communication.
    • A recommended email schedule for participant engagement.
    • A company-specific Clovi registration link with a unique Group ID Code (HIPAA-compliant).
    • A detailed recommendation report for addressing workplace anxiety (Link to A Sample Recommendation Report )
    • A list of the participant's names (no other information due to HIPAA requirements) after the assessment window closes.
  • Participants will receive:
    • A HIPAA-compliant email via Clovi within hours (on business days) that contains a link to log into their account to view their results PDF. This PDF includes their score, symptom insights, and resources dependent on the score.
